If
one of the parents of the registrant has legally changed his or her name,
their name on the registrant's birth certificate can be changed also.
To have this correction made you must provide a certified copy of a court ordered name change. You may bring the court order to the Office of Vital Records in person, or file your request by mail. If you are submitting your request by mail please include and complete an application and send a copy of your valid government issued ID or you can have your signature notarized on the application. Acceptable forms of payment are cashiers check or money order made payable to the Office of Vital Records. Debit/Credit cards are also acceptable.
Walk-in customers are served in the order they arrive between 8:00
a.m. and 4:00 p.m., Monday through Friday. Customers who arrive after
4:00 p.m. will receive information only.
The fee for processing of a court ordered name change is NOT
paid by the Court. The fee includes one certified copy of the
birth certificate with the changed name on it. Please reference the fee schedule for the fee to amend a birth certificate.
