Central Vaccine Recovery Program
 

Information:
The federal government has developed the voluntary “Central Vaccine Recovery Program” to recover unused doses of 2009 H1N1 influenza vaccine.

Provider Information: How to Request 2009 H1N1 Influenza Vaccine

Step 1: Register.
For Arizona healthcare providers who have not already registered to receive novel H1N1 influenza vaccine: The Arizona Department of Health Services will be able to add 1,270 new providers to its vaccine distribution system. All interested providers must complete registration and order forms online.

Step 2: Receive PIN Number from ADHS Immunization Program Office.
ADHS will send providers a unique PIN number. It is extremely important to save this number as it will be required for placing all orders.  VFC Providers will use their VFC PIN number, once they have registered for H1N1 vaccine.  VFC providers will not be automatically enrolled in the H1N1 vaccine ordering/distribution process.

Step 3: Using PIN number, place your order here.

Step 4: Report on a Weekly Basis using the form below.

Vaccine Recall Information
December 22, 2009: VOLUNTARY NON-SAFETY-RELATED RECALL OF SPECIFIC LOTS OF NASAL SPRAY VACCINE FOR 2009 H1N1 INFLUENZA

MedImmune announced that it is voluntarily recalling unused doses of 13 specific lots of Influenza A (H1N1) 2009 Monovalent Vaccine Live, Intranasal due to a slight decrease in potency.
