To get started simply click on the appropriate link below starting with the Premier EMS Agency Participant Handbook, which contains the application. If you would like to send data, please also fill out the Data Submission Application and the User Affirmation Agreement. Once complete, save a copy for your records and email the completed application(s) to Joel Bunis at joel.bunis@azdhs.gov or David Harden at david.harden@azdhs.gov.
